I am having a saving account bearing number [mention your saving number] in your bank’s [mention branch name] branch. On [Date][Time], I was trying to dispense money of Rs [mention amount] from your ATM at  [mention location and address of ATM], having ATM code number [——] No cash has been dispensed from ATM machine even after selecting all required options as on the screen.

However, I received an SMS regarding the deduction of the amount from my savings account. I waited for some time in order to ensure, might be I get a message of reversal of the deducted amount but unfortunately, no SMS was received, even I took a mini statement wherein I checked on the transaction that shows no amount deducted but there was no reversal.

Thus, I kindly request you to look into this matter and credit back the deducted amount [mention amount here] to my account at the earliest.

Thanking you in anticipation for your kind attention!

Yours faithfully,
